Eligibility - Any legal organization domiciled in Louisiana is eligible to apply, organizations with facilities outside of Louisiana may apply if they have physical facilities within Louisiana.
Categories - There are three major categories of organizations ( for administrative purposes only) in the LPEA process:
Award Levels - There are three (3) levels of participation in the Louisiana Performance Excellence Award process. The three levels are designed to support continuous improvement of the applicant’s performance management systems, approaches and results. The level of applicant recognition is based on the applicant’s demonstration of the criteria application at the selected award level.

Fees for 2007 Assessment Award Cycle Level I - Review by Examiners; Feedback Report; On-Site Visit
Level II - Review by Examiners; Feedback Report; and On-Site Visit
Level III - Review by Examiners; Feedback Report; and One-Day On-Site Visit

Intent to Apply Form - The Notice of Intent to Apply Form contains specific information and should contain no more than 2 pages. It will detail: Number of Sites; Organizational Chart; Number of Employees; Applicable Award Category (Level I, II, or III); Address(s) and email; Applicant Contact; Phone Number(s) ; Registrations, Awards Received ; Standards certified to; and other pertinent information Download the Intent to Apply Form for more details.
Criteria - Louisiana's Performance Excellence Award criteria is the same as the Malcolm Baldrige Criteria for business, education, and health care.
